Cancellation Policy
You will receive both email and text message reminders 3 days prior to your consultation time.
If you need to cancel or postpone your session, please give Evolve Psychology and Consulting a minimum of 24 hours’ notice, otherwise our full consultation fee will be charged.
For clients who cancel or postpone between 24-48 hours, 50% of the consultation fee will be charged.
If there are exceptional circumstances that arise that prevent you from attending or notifying us of your inability to keep an appointment, please contact us at info@evolvepsychologyandconsulting.com.au
Disclosure of Personal Information Policy
Personal information gathered as part of the service provided by Evolve Psychology and Consulting will remain confidential except when:
1. it is subpoenaed by a court; or
2. failure to disclose the information would place you or another person at serious risk to life, health, or safety; or
3. your prior approval has been obtained to; or
4. provide a written report to another professional or agency (e.g., a GP or a lawyer); or
5. discuss the material with another person (e.g., an employer or health provider); or
6. disclose the information in another way; or
7. you would reasonably expect your personal information to be disclosed to another professional or agency (e.g., your GP) and disclosure of your personal information to that third party is for a purpose which is directly related to the primary purpose for which your personal information was collected.
